Project Purpose:
The purpose of this project is to help elderly residents live indepentdently, avoid frequent re-hospitalization, and maintain a high quality of life.

Project Service Area:
This project serves the 15,466 elderly residents (age 65 and older) in nine counties: Hall, Boone, Greeley, Hamilton, Howard, Merrick, Nance and Sherman, and approximately 553 elderly residents who reside in Buffalo, Madison, Platte, Valley and Wheeler counties.
