Project Purpose:
The purpose of Ozark Mountain Health Network (OMHN) is to increase access to healthcare for exceptionally rural, vulnerable populations by connecting the current services and implementing services where there are gaps in existing services.  The goals are: 1) Implement a culturally relevant Reach Out and Connect (ROAC) program in exceptionally rural communities that will a) increase the number of rural residents who have a medical home; b) offer free health screenings; c) increase health education and literacy; d) and build a strong coalition of culturally relevant community advocates to ensure continued success, 2) Establish a Certified Diabetes Education Center, and 3) Implement a Prescription Assistance Program.

Project Service Area:
Located in Clinton, Arkansas, OMHN serves the residents of Van Buren and Searcy counties.  These two exceptionally rural counties in the Delta region of Arkansas encompass over 1,300 square miles of a geographically dispersed, impoverished, and underserved population.
