Project Purpose:
The purpose of the project is to improve oral hygiene, improve understanding and treatment of diabetes and other chronic conditions as well as ability of patients to self-manage their conditions, improve continuity of care and reduce financial barriers to care, and advance public policy regarding dental care, patient education, and case management services for low-income and uninsured residents in Texas.
