Project Purpose:
The purpose of this project is to deliver integrated primary and mental health care to isolated, chronically ill population groups in our service area.

Project Service Area:
This project will primarily serve low income adults and children with unmet health care needs, especially those with both chronic conditions (diabetes, hypertension, respiratory illnesses) and mental illness residing in Harrison and Marion Counties.
