Project Purpose:
The purpose of this project is to strengthen the Upper Midlands Rural Health Network and its effectiveness in improving the system of health care in the Network region, reduce absenteeism of middle school students with the chronic conditions of asthma and diabetes, and increase community knowledge of the risk factors for diabetes and asthma and how to manage them.

Project Service Area:
Middle school students, ages 11-15, with asthma and diabetes in the two county Upper Midlands region are the primary target population for this project.
