Project Purpose:
Collaborative Action for Taos County Health (CATCH) continues to be the foundation for regional coordinated health care services.  The purpose of this project is to finalize the implementation of a web based electronic disease state management database that can be accessed by multiple health related services, establish a Health Corps/Promotora program, and expand the CATCH Prescription Assistance Program by formalizing a network of community prescription assistance providers.

Project Service Area:
The service area of this project is Taos, New Mexico, which is home to a tri-cultural population (Hispanic, Anglo, and Native American) that is afflicted by poverty, unemployment, lack of medical insurance, and a health care delivery system marked by fragmentation.
