Project Purpose:
The purpose of the Chronic Care Outreach Program is to reduce health disparities and increase access to nationally recommended healthcare services for the population of the area living with diabetes and other chronic illnesses. 

Project Service Area:
The service area includes the Wheatland Memorial Healthcare (WMH) service area.  WMH is located in Harlowton, Montana, a ranching community in central Montana with approximately 1.4 people per square mile.  The population of the service area is estimated at 4,000 people, with WMH serving residents of Wheatland, Golden Valley, Judith Basin and portions of Sweet Grass and Meagher Counties.
