Project Purpose:
The goals of the project are to reduce the number of hospitalizations resulting from diabetes or diabetic complications in Irwin and Ben Hill counties by 10%, to increase healthy lifestyle behaviors among middle school children and to reduce the incidence of type 2 diabetes through awareness of prevention strategies.

Project Service Area:
The service area of this project includes two counties in Southern Georgia, Ben Hill and Irwin, each of which has a high prevalence of diabetes.  The target population includes three groups:

  1. individuals who have been diagnosed with type 2 diabetes, with an emphasis on those who do not have insurance and/or who live in poverty;
  2. middle school children who need to develop healthy lifestyle behaviors that will lower their risk of becoming diabetic; and
  3. the general public.
