Project Purpose:
The purpose of this project is to increase the quality and years of life for individuals of the target population of un- and underinsured residents of Talladega County, Alabama with chronic diseases of Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Diabetes and/or Hypertension. This project leverages an existing community partnership and uses a computer-assisted Parish Nurse Disease Management Program (PNDMP).
